2011/2012 Jazz Academy Ensemble (Check/Cash)
This is the second installment of the 2011/2012 annual tuition for JAE and JAE-SSF which is due by January 15 -- prior to the 2nd half of term.

The Jazz Academy Ensemble annual registration fee is $530.00. If the 1st installment is due before September 21, 2011 and the second half of $177.00 is due by January 15th.
All notices of withdrawal and request for refund must be made in writing. You may e-mail your request to iambush@jazzacademy.org or fax to 301-871-7294.
Cancellations for any reason 14 days or more prior to the start of JAE rehearsals are eligible for 50% refund and minus the $75 processing fee.
CANCELLATION REQUESTS FOR ANY REASON, LESS THAN 14 DAYS PRIOR TO THE START OF JAE REHEARSALS WILL NOT BE ELIGIBLE FOR A REFUND.
