This is a credit card payment for the second installment of the 2011/2012 annual tuition for JAE or JAE-SSF.
PLEASE INFORM THE JAZZ ACADEMY IF THERE ARE ANY CHANGES THAT WILL AFFECT THIS DEDUCTION! You may also choose to pay the full tuition payment by adding the Spring Term or paying the 2011/2012 full term.

All notices of withdrawal and request for refund must be made in writing. You may e-mail your request to iambush@jazzacademy.org or fax to 301-871-7294.
Cancellations for any reason 14 days or more prior to the start of JAE rehearsals are eligible for 50% refund and minus the $75 processing fee.
CANCELLATION REQUESTS FOR ANY REASON, LESS THAN 14 DAYS PRIOR TO THE START OF JAE REHEARSALS WILL NOT BE ELIGIBLE FOR A REFUND.
