The Maryland Department of Health and Human Services requires that all camp participants complete an emergency medical form. Each student must have one of these forms on file with the Jazz Academy. You must complete the form online or bring it with you on the first day of camp. Failure to do so could result in lost time as we may be forced by the Health Department to temporarily dismiss students who do not have a form on file.

For a hard-copy of the Medical Form and Medication Form if the student will need to take medication(s), including the use of an epi-pen, please request the PDF forms from the office by calling 301-871-8418 or emailing coordinator@jazzacademy.org .
